I hereby authorize Jesus Provides Our Daily Bread (Jesus Center) to transfer the amount listed above from the account listed above on the tenth of each month. This authority will remain in effect until I provide notice to cancel this agreement. I understand this EFT request may take two to six weeks to be processed.
